Steps to Updating Your Infection Records

1. Fill up and submit the Recognition of Overseas COVID-19 Infection Records form, click here.

2. Email us the required documents. We will verify your documents and its authenticity.

3. Once the vaccination certificate has been verified successfully, we will contact you to verify your Name / IC / Passport and Date of Birth.

4. Payment will be made via Paynow / Favepay. Instructions will be given during the call.

5. Your COVID-19 infection records have been updated.

Required Documents

1. Discharge Memo or Recovery Certificate issued by foreign authority or foreign medical practitioner

2. Positive Test Results from PCR test / supervised ART test / Serology test administered outside of Singapore

A discharge memo or recovery certificate issued by a foreign authority or foreign medical practitioner indicating the confirmation of the COVID-19 infection of the patient through either of the following:

i.   a positive test result from a PCR test administered outside of Singapore; OR

ii.  a positive test result from an ART supervised or administered outside Singapore; OR

iii. a positive test result from a serology test administered outside Singapore that is not attributable to the administration of any vaccine against a COVID-19 infection.

A positive test result from an ART supervised or administered outside Singapore, that is certified by a foreign ART provider, foreign medical practitioner or foreign healthcare institution.*

*For avoidance of doubt a positive test result from a self-administered ART is not accepted.

A positive test result from a serology test administered outside Singapore that is not attributable to the administration of any vaccine against a COVID-19 infection.

*If the authenticity of the documents is in doubt, the doctor has the discretion to reject/and or discuss with the patient, and request for him/her to undergo a local confirmatory serology test.
