Prepare Form & Consult Our Doctor
Schedule an appointment to discuss your AMD and its implications. For completing the form, you have two clear options:

Prefill at Home: You can download the AMD form and complete it prior to your visit.

Doctor Assisted: Our doctors can assist you with filling the form during your consultation.

Sign Your AMD Form
You will sign the official AMD form in the presence of our doctor and one additional witness. Our clinic facilitates this process.

Official Registration with MOH
Submit the form to the Registrar of Advance Medical Directives at the Ministry of Health by mail.
Once registered, your AMD becomes a legally binding document.
