Since graduation, he has done several postings in restructured hospitals for different aspects of medical care such as renal medicine, obstetrics & gynaecology, neonatal, geriatric, dermatology, respiratory and orthopaedic medicines. As a general practitioner, he has worked in a few reputable medical groups for many years and accumulated a wealth of clinical experiences. Dr Lye was a tutor for the students in Family Medicine to the students in NUS. Dr Lye is well known for his humorous, opened and caring approach to his patients. He is a friendly and humble doctor who is committed to providing his patients with the highest quality and standard of care to the best of their expectations. Dr Lew Hong Jian graduated in 1998 with his medical & surgical degree from National University of Singapore. An experienced and well-trained family physician, Dr Lew has 27 years of clinical experience in family medicine and general surgery. He holds a qualification of Diploma in Family Medicine and he specializes in internal medicine. Since graduation, he has worked in a number of disciplines in various hospitals for a different aspect of medical care such as internal medicine, ear nose & throat, and geriatrics care etc. Dr Lew is well known to his patients as a friendly, caring and humble doctor. Having graduated from the National University of Singapore in 1997, he has undergone various postings in the hospital including the Accident and Emergency Department, General Medicine, General Surgery, Psychiatry, Neonatology etc. He spent most of his training in the polyclinic and holds a Post-Graduate Diploma in Family Medicine. Dr Lam is an adjunct lecturer at the School of Health Sciences, Nanyang Polytechnic since 2005. He enjoys tutoring and sharing of ideas and knowledge with his students as well as his patients. Dr Mohd Saifuz graduated in 1995 with his medical and surgical degree from National University of Singapore. An experienced and well-trained family physician, Dr Mohd Saifuz has 30 years of clinical experience in family medicine. Since graduation, he has practiced in various government restructured hospitals and working in polyclinics gaining a wealth of his experience in Family Medicine. He obtained his Graduate Diploma in Family Medicine from the National University of Singapore in 2008 and is accredited as a Family Physician by the College of Family Physician Singapore. His area of interest is in Internal Medicine, Chronic Disease Management, Preventive Health and Children’s Health. Dr Mohd Saifuz is a friendly, caring and humble doctor. Since then, she has undergone extensive clinical training across multiple disciplines, including Internal Medicine, General Surgery, Orthopaedic Surgery, Cardiology, and Anaesthesiology, equipping her with a strong and comprehensive foundation in patient care. Dr Tan is currently pursuing her Graduate Diploma in Family Medicine (GDFM) and is an accredited Lasting Power of Attorney (LPA) Certified Issuer. She is known for her friendly and gentle demeanour, meticulous clinical approach, and unwavering commitment to providing attentive, thoughtful, and patient-centred care. Dr Jaclyn Chan graduated from University College Cork for her MBBS and The University of Edinburgh for her master’s degree in Ophthalmology. She brings us a new clinical foundation and diverse medical experience. She previously served at Pinnacle Family Clinic and Crawfurd Medical, where she was actively involved in as a resident doctor. Dr Jaclyn is currently pursuing her Graduate Diploma in Family Medicine (GDFM) on a part-time basis. Her clinical postings have equipped her with broad and valuable exposure across multiple specialties, including colorectal surgery, geriatric medicine, orthopaedics, general surgery, neurosurgery, and internal medicine. She also holds a master’s degree in Ophthalmology and a Minor in Psychology, reflecting her commitment to continuous learning and her holistic approach to patient care. She has been accredited under the PaedsENGAGE program conducted by KKWCH and NUH to manage non-emergency paediatric cases to ensure continuity of care.
