Terms and Conditions (Remote Overseas Vaccination Records) All individuals who have been vaccinated overseas with any vaccines approved under the WHO Emergency Use Listing (EUL), including booster shots, can get their overseas vaccination records ingested into the National Immunisation Registry (NIR). If their overseas vaccination documents are not in English, they will need to get the documents translated by a translation service provider, notary public or embassy of the country that issued the document. Both the original overseas vaccination document and translated document must be shown for verification. Holders of the following types of certificates may be exempted from serology testing for their vaccinations to be recognised. However, your certificate must be able to be scanned to verify its authenticity. If you are a job applicant, personal data which we may collect includes, without limitation, your: (a) name or alias, gender, NRIC/FIN or passport number, date of birth, nationality and country/city of birth; (b) mailing address, telephone numbers, email address and other contact details; (c) résumé, educational qualifications, professional qualifications and certifications, and employment references; (d) employment and training history; (e) work-related health issues and disabilities; and (f) photographs. 4. If you are an employee, personal data which we may collect in the context of your employment with us includes, without limitation, your: (a) name or alias, gender, NRIC/FIN or passport number, date of birth, nationality and country/city of birth; (b) mailing address, telephone numbers, email address and other contact details; (c) employment and training history; (d) salary information and bank account details; (e) details of your next-of-kin, spouse and other family members; (f) work related health issues and disabilities; (g) records on leave of absence from work; (h) photographs and other audio-visual information; (i) performance assessments and disciplinary records; and (j) any additional information provided to us by you as a job applicant (i.e. prior to being engaged as an employee). 5. Generally, Company may collect personal data in various ways including: (a) when you submit an employment application or provide documents or information such as your résumé with your employment and training history, your personal particulars which includes salary information and bank account details, photographs, details of your next-of-kin, spouse and other family members and employment references; (b) from public information sources, search services and other third parties; (c) when you submit your personal data to us for any other reason; (d) CCTV recordings while you are within our premises. The CCTV cameras are placed at strategic locations such as door entrance/exit, reception areas, common walkways and locked storage areas. In addition to security reasons, the CCTV footages will be disclosed to the management for employees’ code of conduct, disciplinary and enforcement purposes; (e) door access logs of employees. Entrance and exit to and from the office premises is controlled by a door access system using either biometric fingerprints or access cards. In addition to security reasons, the recordings of door access logs of employees will be disclosed to the management for code of conduct, disciplinary and enforcement purposes; (f) when you attend a seminar or other event organised by us; (g) photographs or videos of you and/or any other accompanied persons (e.g. your family members or friends) taken by us or our representatives when you attend events hosted by us, company trips and family day events; and (h) if you provide us with any personal data relating to a third party. By submitting such personal data to us, you also represent to us and must ensure that you have notified the third party of the terms of this Policy and obtained his consent thereto. 6. Other terms used in this Policy shall have the meanings given to them in the PDPA (where the context so permits). COLLECTION, USE AND DISCLOSURE OF PERSONAL DATA 7. The purposes listed in the above clauses may continue to apply even in situations where your relationship with us (for example, pursuant to a contract) has been terminated or altered in any way, for a reasonable period thereafter (including, where applicable, a period to enable us to enforce our rights under any contract with you). WITHDRAWING CONSENT 11. The consent that you provide for the collection, use and disclosure of your personal data will remain valid until such time it is being withdrawn by you in writing. The test is valid for not more than 24 hours before the end of the event. ART is a self-paid test and does not qualify for government subsidies if you do not have ARI symptoms. It takes about 15 minutes. Pre-Departure Travelling Antigen Rapid Test is at $25.00, with notarised digital health certificate generated for Departure. Travellers are responsible to know their own requirements for the VTL countries they are travelling to, and allocated sufficient time to undergo the test and receive your results in order to meet the requirements of your transiting airport or travel destination. Tele-ART is available at $25.00 for anyone who wishes to do their Pre-Departure Testing and get their results without having to leave the house. What to Prepare for our Supervised Tele-ART Step 1. Identity Verification Present your NRIC or Passport and Travel Itinerary(For Travels) to your Supervisor for verification before starting. Step 2. Test Kits Verification Do prepare 2 sets of ART test kits (1 for back-up purpose). Present your unopened test kits showing the Batch/Lot number to your Supervisor for verification. Step 3. Test Kits Preparation Set all the components up on a table or flat surface (3 to 5 components) before doing your swab. Ensure your test kits are always in the camera's view. Adjust if necessary before the start of the test. Step 4. Conduct Self-Administered ART Our Supervisor will guide you through a list of instructions for your self-test ART. Do remember that your test must always be in the camera's view. Step 5. Waiting and Present your Result Leave your video on for 15 minutes for the results. Once the time is up, hold up your test result and present it to our Supervisor for verification. *In any event of results being inconclusive, do follow our Supervisor's instructions and repeat from Step 2 to 5 with the back-up test kit. *In any event of disrupted connection or non-continuous real time remote supervision, your test will be considered invalid, you will be required to repeat from Step 2 to 5 with the back-up test kit.
