    Terms and Conditions (Tele-ART) Please ensure that you have read and understood all the following requirements before proceeding with your Remote Supervised Antigen Rapid Test (Tele-ART). If you have frequent nose bleeds, bleeding disorders, recent surgery or trauma to your nose or nasopharyngeal cancer we will not be able to supervise the Tele-ART Swab Test. If you have any symptoms, please consult a doctor for physical consultation. Remote Supervised Antigen Rapid Test results may return as inconclusive: In that event, you are required to repeat the test, do follow our Supervisor's instruction for a repeat test from Step 1 to 4 as referred below. Should this result affect your departure time, Etern Medical Clinic and/or the appointed laboratories vendor(s) shall not be held responsible or liable for any claims, damages, losses, expenses cost or liabilities. Preparation for Remote Supervised Antigen Rapid Test 1. Present your NRIC or Passport and Travel Itinerary (For Travels) to our Supervisor for verification. 2. Prepare 2 sets of ART test kits (1 for back-up purpose). Present all the test kits showing the Batch/Lot number to our Supervisor and set all the components on the table or flat surfaces*. 3. Follow our Supervisor's instructions and do your ART self-test accordingly. Our Supervisor will guide you if you have any enquiries. 4. Wait for 15 minutes for the results to show. Once the time is up, present your result to our Supervisor for confirmation. During the 15 minutes wait, you may leave your camera to do your thing*. *Always ensure that your test is in the camera's view. Adjust accordingly before starting the self-test. **In any event, your result is inconclusive, do follow our Supervisor's instruction and repeat the process with your back-up ART test kit. *** In any event of disrupted connection or non-continuous real time remote supervision, your test will be considered invalid, you will be required to repeat the test from Step 1 to 4. Delivery of Results You will receive your results within 1 to 2 hours after your test via HealthHub or TraceTogether. For Travellers, you will receive your Notarised Digital Health Certificate via the email you provided. You are advised to print out and bring along the hardcopies of your Notarised Digital Health Certificate with your overseas. By reading this agreement, I agree to the following conditions set out: 1. I have checked the timeframe requirements of the destination countries I am travelling to, the travel lanes and the PCT testing window frame. I will not hold Etern Medical and the laboratory liable for any additional costs incurred should there be any rejections from the airlines, embassy, and authorities due to the wrong appointment made that is based on the country's pre-arrival swab requirements time. 2. I acknowledge that Etern Medical Clinic accepts no responsibility or liability for any missed flights due to inconclusive results or any other unpredictable factors that may occur. 3. I understand that if my pre-departure test result returns as positive, I am not permitted to travel and MOH will be notified of my results. 4. I agree that Etern Medical may collect, use and disclose my personal data to contact me in accordance with Personal Data Protection Act (PDPA) 2012 and Etern Medical's data protection policy. 5. I declare that all information provided here by me is accurate, complete and true. There will not be any refund for cancellation.     Our clinics offer various government schemes for our patients, such as CHAS, Medisave, Screen For Life (SFL), and many more. Government Schemes Your Guide to Subsidised Care Medisave Medisave was designed to help Singaporeans pay for their medical expenses on chronic conditions and vaccinations at GP clinic. Under this scheme, the patient of their immediate family members can withdraw up to $500 (simple chronic condition) or up to $700 (complex chronic condition) per patient per year to pay for the following outpatient medical expenses: Flexi-Medisave If you're a Singapore Citizen or Permanent Resident aged 60 or above, you can use up to $400 of your Medisave each year to pay for a variety of outpatient medical treatments. This includes: Consultations Screening tests Vaccinations Medication To use your Flexi-Medisave, simply let our staff know when you are at the clinic. Medisave Grant for Newborn Newborn citizens will get a Medisave account with a grant of $5,000* in their Medisave account. This grant can help parents defray the costs of their child’s MediShield premiums , as well as other healthcare expenses such as vaccinations. *For children born on or after 1 April 2025. Medisave Claimable for Adult Vaccinations A new list of adult recommended vaccines are also Medisave claimable with effect. (For persons aged 18 years and older as recommended by MOH) Influenza Pneumococcal Cervical Cancer vaccination Tetanus,diphtheria & pertussis Measles,mumps & rubella (MMR) Hepatitis B Chicken Pox Chronic Disease Management Program Diabetes Hypertension Stroke Lipid Disorder (Cholesterol) Asthma Dementia Bipolar order Major Depression Chronic Obstructive Pulmonary Disease Parkinson Disease Osteoarthritis Gout Chronic Hepatitis B Allergic Rhinitis Vaccinations Program for Adults Singaporeans can use their Medisave to pay for vaccinations such as pneumococcal and influenza, Cervical Cancer and Hepatitis B vaccinations. For cervical cancer vaccination, the approved age for Medisave usage is for ladies age between 9 to 26 years old. Our clinics carry both Gardasil and Cervarix vaccines which are approved for the prevention of disease caused by Human Papillomavirus (HPV). Community Health Assist Scheme (CHAS) At Etern Medical, we are able to manage patients with chronic conditions using both CHAS card and CDMP program. Our patient is able to use their Medisave for the outpatient treatment of the same set of chronic conditions covered under CHAS. CHAS cardholder will receive subsidies for treatment of chronic conditions and/or common illnesses and selected dental services at participating CHAS clinics. Do remember to bring along your CHAS card when visiting our clinic. Baby Bonus Program Our medical bills can be paid through the Child’s Development Account (CDA). Parents can use their Baby Bonus cash gift and/or savings in their Child Development Accounts (CDA) to pay for all consultations, procedures, vaccinations, including pneumococcal and hepatitis B vaccinations, at Baby Bonus approved medical institutions. The Baby Bonus and CDA can also be used to pay for siblings’ vaccination. To register your child for this scheme, please come down to our clinic with the following documents: Your NRIC if you are the trustee of the child Your child’s Birth Certificate Your child’s Baby Bonus NETS card Medical Expenses Ministry of Health has approved the use of Medisave $500/$700 per account per annum scheme for the vaccinations for those in high-risk group and some medical conditions. All childhood vaccinations under the NCIS (National Childhood Immunisation Schedule) for Medisave is claimable up to $500/$700 per annum per account holder. Parents can also choose to pay with the combination of baby bonus and Medisave. Teleconsult is for patients that have any simple non-assessment ailments such as: Patients with simple conditions that do not need physical examination or further testing for management. Patients who are generally well and only has minor symptoms that doesn’t need much medication but require medical rest. (E.g., Headache, Body ache, Back pain, etc.) Patients who have minor illnesses that they have prior experience of and requires only some simple medications that they have taken before. Patients with existing chronic condition who are well and require a simple consultation and refill of the same medications. What medical conditions are unsuitable for teleconsult services? Patients with uncontrolled chronic conditions or mental health concerns are advised against using teleconsultation. Symptoms listed below are also considered unsuitable for teleconsult, and may require immediate medical attention at the Accident & Emergency department of the nearest hospital: Acute or severe chest pain Actively bleeding or deep wounds or lacerations Breathlessness Convulsions or seizures Fainting spells, presyncope or syncope Injuries with suspected fractures or dislocations presenting as severe pain, open wounds, deformity, severe bruising or swelling, loss of sensation, and limb weakness Persistent vomiting Severe abdominal pain Severe headache or giddiness Sudden onset numbness, weakness or slurred speech Sudden loss of vision Are there any age limitations for teleconsult? Yes. Teleconsult is limited to patients aged 16 years and above. Patients who are aged 16 and below shall be accompanied by an adult throughout the whole teleconsult. Are there any preparations to be fulfilled prior to my teleconsult appointment? Ensure that you have the WhatsApp Messenger application downloaded in your phone and logged in. Where possible and relevant, have photographs of your condition, e.g. infection, skin problem or wound, recent medical records and/or latest laboratory results. Do send the relevant images through WhatsApp Messenger to the clinic’s mobile phone number before commencing the teleconsultation to facilitate the doctor’s review. Where available and medically appropriate, it is strongly recommended to be prepared with the following equipment on hand during the teleconsultation: - Thermometer - Blood pressure machine - Glucometer (for diabetic patients) - Weighing scale - Torchlight You may record your vitals before your teleconsultation and have them ready for your doctor. Be ready to verify your name and identity or passport number with your doctor before commencing the teleconsultation. Do have your identity card or passport ready on hand as well. What are the costs involved? Our consultation fees are exclusive of GST, the cost of medication, and any additional charges such as memos or referral letters, as well as administrative and delivery fees (where applicable). At the end of your teleconsultation, you will be issued an itemized invoice detailing the total charges incurred, which will include the following: Consultation fees GST Cost of medication (if applicable) Memo or referral letter (if applicable) Administrative charges Delivery charges (if applicable). What if the connection is lost in the midst of the teleconsult? Our doctor will give you a return call if the connection is lost during the teleconsult. Should there be any technical difficulties and a re-connection is not possible, our clinic nurse will contact you again shortly. However, we would strongly recommend that the signal strength of the mobile data or Wi-Fi connection is stable before starting the teleconsult. This would help to minimize connectivity issues. Am I able to choose my doctor? Patient can inform the clinic his/her preferred doctor. However, this must be clearly stated during appointment booking and is still further subjected to the availability of the preferred doctor. The medications will be dispensed from the clinic the doctor is currently stationed at. Can I get a Medical Certificate? Yes, you may request. However, a medical certificate is not always guaranteed. The doctor will assess and recommend the best course of treatment which may or may not involve the issuance of a medical certificate. The medical certificate can be retrieved either via the eHealthAssist (eHA) app or emailed to you in PDF format or you may also opt for self-collection for the hardcopy medical certificate at the clinic premises. Can I get a medical prescription? Yes, you may request. This is however subjected to the doctor’s assessment & management of the condition, which may or may not include the exact medical prescription that you have requested. Do note that an additional charge of $15 for a medical prescription letter applies. There is no doctor’s signature on the digital MC. How do I prove that the digital MC is authentic? As the digital MC is a computer-generated MC, the doctor’s signature is not required. The doctor’s signature is in the form of his / her name block letters on the digital MC together with the doctor’s MCR number. If you need further clarification, please call the clinic. What should I do if I missed my scheduled teleconsult appointment? If you missed your scheduled appointment, you may call the clinic phone or send a WhatsApp message to the respective clinic mobile number and the nurses will be able to assist you.
