Terms and Conditions (Remote Overseas Vaccination Records) All individuals who have been vaccinated overseas with any vaccines approved under the WHO Emergency Use Listing (EUL), including booster shots, can get their overseas vaccination records ingested into the National Immunisation Registry (NIR). If their overseas vaccination documents are not in English, they will need to get the documents translated by a translation service provider, notary public or embassy of the country that issued the document. Both the original overseas vaccination document and translated document must be shown for verification. Holders of the following types of certificates may be exempted from serology testing for their vaccinations to be recognised. However, your certificate must be able to be scanned to verify its authenticity. What to Prepare for our Supervised Tele-ART Step 1. Identity Verification Present your NRIC or Passport and Travel Itinerary(For Travels) to your Supervisor for verification before starting. Step 2. Test Kits Verification Do prepare 2 sets of ART test kits (1 for back-up purpose). Present your unopened test kits showing the Batch/Lot number to your Supervisor for verification. Step 3. Test Kits Preparation Set all the components up on a table or flat surface (3 to 5 components) before doing your swab. Ensure your test kits are always in the camera's view. Adjust if necessary before the start of the test. Step 4. Conduct Self-Administered ART Our Supervisor will guide you through a list of instructions for your self-test ART. Do remember that your test must always be in the camera's view. Step 5. Waiting and Present your Result Leave your video on for 15 minutes for the results. Once the time is up, hold up your test result and present it to our Supervisor for verification. *In any event of results being inconclusive, do follow our Supervisor's instructions and repeat from Step 2 to 5 with the back-up test kit. *In any event of disrupted connection or non-continuous real time remote supervision, your test will be considered invalid, you will be required to repeat from Step 2 to 5 with the back-up test kit.
